Description: This course provides a comprehensive review of Müllerian anomalies, with a particular focus on obstructive anomalies and their surgical management. Using a case-based discussion format, participants will review the embryology of these conditions as well as approaches to management during early puberty, including imaging, diagnostic workup, and treatment of uterine and vaginal obstruction. The course will include discussion of diagnostic strategies such as advanced imaging and examination under anesthesia to achieve accurate diagnosis. Particular attention will be given to distinguishing complex conditions—for example, differentiating OHVIRA (Obstructed Hemivagina and Ipsilateral Renal Anomaly) from unilateral cervicovaginal agenesis or dysgenesis in the setting of a didelphys uterus. We will review surgical management including specific laparoscopic and hysteroscopic resection techniques, as well as vaginoplasty procedures performed with and without grafts. The course will additionally address ethical considerations regarding the timing of reconstructive surgery. This session will be of interest to minimally invasive gynecologic surgery (MIGS) specialists as well as general obstetrician–gynecologists. It will be presented by pediatric gynecology experts from across North America with extensive experience managing these conditions in childhood and facilitating transition to adult care.

Learning Objectives: At the end of this course, the participant will be able to:

  1. Improve recognition and diagnostic accuracy of complex Müllerian anomalies;
  2. Review appropriate imaging and evaluation strategies; and
  3. Discuss surgical management, including laparoscopic, hysteroscopic, and reconstructive techniques.
