Description: Uterine fibroids remain the most common benign tumors of the female reproductive tract and a leading indication for gynecologic surgery worldwide. Despite the frequency of diagnosis, optimal management remains complex and highly individualized. Advances in minimally invasive surgery, hysteroscopic technology, radiofrequency ablation, uterine-sparing interventions, and robotic platforms have expanded therapeutic options but also increased decision-making complexity.
This postgraduate course provides a comprehensive, evidence-based review of the contemporary management of the fibroid uterus. Emphasis will be placed on patient selection, symptom-driven treatment algorithms, fertility considerations, risk of recurrence, surgical planning, and complication avoidance. Faculty will discuss office-based strategies, hysteroscopic and laparoscopic techniques, myomectomy versus hysterectomy decision-making, energy-based therapies, and emerging technologies. Real-world case discussions and video-based surgical analysis will highlight operative judgment, technical pearls, and strategies to optimize outcomes while minimizing morbidity.
Learning Objectives: At the end of this course, the participant will be able to: 1) Classify fibroids using current FIGO systems and correlate anatomy with symptomatology and treatment selection; 2) Develop individualized management algorithms based on bleeding profile, pain, fertility goals, and patient preference; 3) Compare surgical and non-surgical treatment modalities, including hysteroscopic resection, laparoscopic/robotic myomectomy, radiofrequency ablation, uterine artery embolization, and hysterectomy; 4) Identify risk factors for recurrence and strategies to reduce repeat intervention; and 5) Apply technical principles to minimize complications such as hemorrhage, adhesion formation, and conversion to laparotomy.
