Description: As cancer survival improves in young women, fertility preservation has become an essential component of gynecologic oncology care. This surgical tutorial will provide a practical and evidence-based overview of contemporary fertility-preserving strategies in patients undergoing treatment for gynecologic malignancies or pelvic irradiation. International experts will discuss patient selection, surgical techniques, multidisciplinary coordination, oncologic safety, and reproductive outcomes. Topics will include ovarian tissue cryopreservation and oocyte preservation prior to chemoradiation, conservative hysteroscopic management of endometrial hyperplasia and early endometrial cancer, uterine transposition to preserve reproductive potential before pelvic radiation, and ovarian transposition techniques to maintain ovarian hormonal and reproductive function. Through expert discussion and surgical insights, attendees will gain a comprehensive understanding of modern fertility-preserving approaches in complex oncologic settings.

Learning Objectives: At the end of this course, the participant will be able to:

  1. 1. To review current fertility-preserving surgical strategies in patients with pelvic malignancy or requiring pelvic irradiation;
  2. To understand the indications, techniques and limitations of ovarian and uterine transposition; and
