Design: We performed a retrospective chart review of patients at a single academic institution undergoing surgical management with a pre-procedure diagnosis of gonadal torsion over a ten-year period (12/13/12-12/12/22)

Patients or Participants: 86 patients (32 female, 54 male) after meeting inclusion criteria

Measurements and Main Results: The average time from presentation to operating room (TPOR) and average time from imaging results to operating room (TIOR) were significantly greater for females compared males (p = 0.0002 and p = 0.0010, respectively), however the rate of gonadal loss between females and males was not different (p = 0.6398). Among the female patients, there were no differences in TPOR or TIOR based on race, ethnicity, zip code, insurance, or surgeon specialty. Employed female patients had a longer TPOR compared to unemployed patients (p = 0.0492), but no difference in TIOR (p = 0.4554). Among the male patients, there were no differences in TPOR or TIOR based on race, ethnicity, insurance, employment, or surgeon specialty. Both TPOR and TIOR were longer for male patients from a high risk zip code (p = 0.0484 and 0.0245, respectively), defined as the 7 zip codes with the highest poverty rate in metropolitan Cleveland. There was no statistically significant difference in rate of gonadal loss within either female or male patients based on race, ethnicity, zip code, insurance, employment, or surgeon specialty.
Conclusion: Disparities may exist in timing of surgical management but not outcomes of male patients with testicular torsion from a high risk zip code. However, disparities were not apparent in timing or outcomes of surgical management of gonadal torsion within male and female patients based on race, ethnicity, insurance, employment, or surgeon specialty.
