11366 - Surgical Impact on Quality of Life in Endometriosis Patients Using the Endometriosis Health Profile (EHP-30) Questionnaire

Abstract
Study Objective: To evaluate the surgical impact on Health-Related Quality of Life (HRQoL) in patients diagnosed with endometriosis.

Design: Prospective, observational research study comparing EHP-30 scores before and after robotic-assisted laparoscopic surgery in patients with endometriosis.

Setting: Participants were recruited from a private practice office setting, with surgeries performed at three different community hospitals.

Patients or Participants: Patients exhibiting symptoms suggestive of endometriosis were recruited from May 2023 to April 2024. Exclusion criteria included a lack of histopathologic diagnosis of endometriosis on surgical pathology. Initially, 33 patients were recruited with 9 subsequently excluded due to 5 lacking endometriosis on pathology, 1 surgery cancellation and 3 lost to follow-up. After exclusions, 24 participants remained and were analyzed.

Interventions: After completion of the initial EHP-30 questionnaire, patients underwent robotic-assisted laparoscopic excision of endometriosis. Patients with a positive histopathologic diagnosis of endometriosis completed the EHP-30 again at 2 weeks and 12-20 weeks after surgery to assess quality of life.

Measurements and Main Results: The EHP-30 evaluates the effectiveness of medical or surgical treatment of endometriosis on HRQoL containing five scale scores: pain, control and powerlessness, social support, emotional well-being, and self-image. Our preliminary data indicates a statistically significant reduction in EHP-30 total scores 2 weeks post-surgery (p < 0.001) and 12-20 weeks post-surgery (p < 0.001) compared to scores before surgery. Additionally, there were statistically significant reductions in all five individual scales: pain (p < 0.001), control and power (p < 0.001), emotional (p < 0.001), social support (p < 0.001), and self-image (p < 0.001) scores. Conclusion: Surgical excision of endometriosis significantly improves overall quality of life in patients with histopathologically proven endometriosis assessed by the EHP-30 questionnaire. Additionally, significant improvements were noted across all five domains assessed by the EHP-30 questionnaire.
